The position emphasizes daily living assistance, documentation, and teamwork with the care team to promote health, safety and meaningful participation in activities.

Qualifications

• High School diploma or equivalent is required.
• Additional coursework in human services is a plus.
• Experience with individuals with developmental disabilities preferred.
• CPR/First Aid certification or willingness to obtain.
• Proficient computer skills and ability to use an EHR system.

Responsibilities

• Provide compassionate, person-centered care in home or community settings.
• Assist with daily living activities including hygiene and meals.
• Support participation in social activities and personal goals.
• Maintain accurate documentation and communicate with care team.
• Ensure a safe, clean and supportive environment.
