Job summary

Prismhr Hire is seeking a compassionate caregiver to provide high-quality support to residents in a safe, respectful environment in New York. You will assist with daily living activities, promote comfort, and help create a positive experience for each resident.

The ideal candidate is patient, attentive, and dedicated to person-centered care, able to follow care plans, maintain dignity, and collaborate with nursing staff to support well-being.

Qualifications

• High school diploma or equivalent preferred.
• Experience in resident care or home care is helpful.
• Ability to communicate clearly and respectfully with residents, families, and team members.
• Strong interpersonal skills, empathy, and a genuine desire to help others.
• Ability to follow care plans and established procedures.
• Comfort with providing physical assistance, including lifting and mobility support as allowed.
• Basic observation and documentation skills.
• Reliability, professionalism, and confidentiality.

Responsibilities

• Assist residents with activities of daily living, including bathing, dressing, grooming, and mobility support.
• Provide companionship and emotional support while maintaining resident dignity and privacy.
• Monitor and report changes in resident condition, behavior, or needs to appropriate team members.
• Support mealtime assistance, hydration, and nutritional needs as directed.
• Help maintain a clean, safe, and comfortable living environment.
• Follow care plans, safety procedures, and facility policies at all times.
• Assist with transfers, positioning, and use of mobility equipment as needed.
• Document care activities and observations accurately and in a timely manner.
• Work collaboratively with nursing staff, supervisors, and other team members to support resident well-being.
