Job summary

Health at Home is seeking a compassionate Home Health Aide to support patients at home, helping them live with dignity and independence.

Under guidance of a nurse or therapist, you will provide personal care, observe changes, document care, and ensure safety during every visit. Requirements include CPR, age 18+, and preferred 1 year home health/CNA experience.

Travel is required; continuing education and training opportunities are provided to support growth within the care team.

Qualifications

• Must meet the 75 hour training requirement and competency per CFR484.80.
• Certified Nursing Assistant (CNA) may be hired as Home Health Aide and governed by same functions.
• Minimum 1 year hands-on work experience preferred.
• At least 18 years of age.
• Ability to read and follow written instructions and document care.

Responsibilities

• Provide personal care including baths, hygiene, dressing, toileting, and skincare.
• May prepare light meals and assist feeding.
• Take and record vital signs as ordered.
• Assist in ambulation and prescribed exercises.
• Assist in self-administration of medications as appropriate.
• Perform ROM and simple procedures as ordered.
• Report changes to the assigned nurse and ensure safety.
