Qualifications

• Current and unrestricted Illinois Licensed Practical Nurse (LPN) license.
• Graduate of an accredited Practical Nursing program.
• Current CPR/BLS certification preferred.
• Previous skilled nursing, rehabilitation, long‑term care, or similar healthcare experience preferred.
• Current and unrestricted Illinois Registered Nurse (RN) license.

Responsibilities

• Provide direct nursing care to residents in accordance with physician orders and established plans of care.
• Administer medications and treatments accurately and timely.
• Perform resident assessments and identify changes in condition.
• Notify physician, supervisor, and responsible party of significant changes in condition.
• Monitor residents after falls, incidents, hospital returns, new medications, treatments, and changes in condition.
• Complete required nursing documentation accurately and within established timeframes.
• Maintain accurate MARs and TARs.
• Provide wound care and other skilled nursing treatments within the nurse's scope of practice.
• Monitor and manage IV therapy, oxygen therapy, enteral feeding, catheter care, ostomy care, and other clinical services.
