Job summary

SEARHC in Alaska is seeking a coordinator to oversee grant-funded prevention services, including education, screening, and lifestyle counseling, within outpatient care. This role manages grant finances and ensures program compliance across federal/state/tribal funding sources.

Qualifications

  • Master of Public Health or other professional health degree (or Bachelor’s + 2 years related experience).
  • 3 years’ experience in healthcare, public health, nutrition, or related field with grant coordination or management, health education, motivational interviewing or lifestyle counseling services in a public health or ambulatory care setting.
  • 2 years supervisory experience.
  • Knowledge of cardiovascular health, cancer prevention and screening, diabetes prevention and control, tobacco control and cessation, physical activity promotion, and nutrition, including knowledge of health education core competencies and stages of change theory.
  • Advanced knowledge of SEARHC data systems and strong computer skills.

Responsibilities

  • Oversee grant-funded prevention services including screening, education, and lifestyle counseling integration.
  • Oversee grant expenses and provide recommendations to ensure fiscal responsibility for operating budgets funded by federal, state, and/or tribal sources.
  • Supervise staff delivering health education and case management and ensure data entry into Cerner and registries/tracking systems.
  • Collaborate with SEARHC departments and partners to improve access to prevention and healthcare services and coordinate program activities and reporting.
  • Prepare annual reports, non-competing continuation and competitive grant applications, and other service indicator reporting needs with Finance and Grant teams.

Job description

Pay Range:$38.36 - $53.82To coordinate the provision of grant-funded clinically integrated primary and secondary prevention services. This includes overseeing the integration of screening, patient education, and lifestyle counseling into the current outpatient healthcare system.

This position will have oversight of grant expenses and will provide recommendations to ensure fiscal responsibility for the operating budgets of federal, state, and/or tribal funding sources.

Responsible for oversight of patient screening, patient education and follow-up services data entry into Cerner and other registries/tracking packages utilized, as well as quality assurance review with appropriate Community wellness program staff and appropriate clinic managers and medical executive committee members.

Key Essential Functions and Accountabilities of the Job

Provides oversight of staff who provide health education and case management for patients due for screening, diagnostic services, including patient tracking, and follow-up. Works with appropriate operational leads to ensure quality delivery of care for patients who need treatment services, lifestyle interventions, and/or additional follow-up.

Collaborates with other SEARHC departments and strategic partners to improve patient access to comprehensive prevention and healthcare services including coordination of program activities, documentation of services, eligibility screening, reimbursement for services using grant funds, and expansion of services.

Collaborates with other local, state, and federal agencies involved in the provision of services as appropriate. Works with the Director of Community wellness and Grant Writers and the Finance team to prepare appropriate annual reports, non-competing continuation and competitive applications, and other service indicator reporting needs.

Other Functions

Travel to grantor and partner meetings, and for site and supervisory visits is required.

Supervisory Responsibilities

A supervisor has the responsibility and authority to hire, train, evaluate and discipline subordinates. A supervisor is distinguished from a lead person who assigns and checks work, trains, and schedules subordinates daily, and performs the same or more difficult duties performed by the subordinates.

Additional Details:
Education, Certifications, and Licenses Required

Master of Public Health or other professional health degree. A Bachelor’s degree + 2 additional years of related experience can be exchanged for a Master’s degree.

3 years’ experience in healthcare, public health, nutrition, or related field(s) to include grant coordination or management, health education, motivational interviewing or lifestyle counseling services in a public health or ambulatory care setting, including working within complex systems of patient care and referral.

Basic Life Support
Experience Required

2 years supervisory experience.

Knowledge of

Advanced knowledge of specific health topics, including cardiovascular health, breast, cervical and colorectal cancer prevention and screening, diabetes prevention and control, tobacco control and cessation, physical activity promotion, and nutrition, including knowledge of health education core competencies, and stages of change theory, motivational interviewing, and the socioecological model.

Working knowledge of group dynamics, ability to facilitate/train groups, and good customer service skills.

Skills in

Advanced computer application skills including email, word processing, spreadsheet, database, and document organization.

Excellent oral/written communication skills, including interpersonal skills in relating to patients and staff in a sensitive and culturally competent manner.

Superb organizational and prioritization skills.

Problem analysis and solution skills as related to complex health care settings, using a systems approach.

Proficiency in the use of SEARHC’s electronic health data system (preferred).

Ability to

Ability to provide guidance and leadership to all levels of clinic staff, including communication skills to assertively work integrated service processes into existing clinic protocols.

Ability to collaborate with other programs to maximize service delivery and avoid duplication of services.
