Job summary

GT Independence is seeking a Team Leader for the Community Supported Living program in Green Bay, WI. The role provides hands‑on support to clients and mentors Direct Support Staff, ensuring adherence to care plans and program standards.

Responsibilities include directing staff, coordinating with managers and case managers, maintaining documentation, and building relationships with clients and community partners. On‑site work with occasional on‑call duties is required.

Qualifications

• 2 years of relevant experience

Responsibilities

• Provides daily direction to Direct Support Staff
• Follows written plan of care for each member in the program
• Provides hands‑on direct support to members of the CSL program
• Collaborates with CSL Manager, Case Managers, and natural supports to complement member’s plan of care
• Monitors workloads of the group and re‑assigns tasks appropriately
• Completes necessary documentation for each member served
• Responsible for staff training in conjunction with the CSL Manager
• Builds and maintains relationships with members served, funders, and community stakeholders
• Participates in On‑Call rotation responsibilities
• Applies GT Independence values during all workplace practices
• Other duties as assigned
