Elwyn is seeking a Community Support Worker to train, counsel, and supervise individuals in activities of daily living and community inclusion, adhering to Individual Service Plan goals and documenting progress.
You will work in home and community settings providing one-to-one supervision, assist with medical appointments, and help participants achieve ISP objectives while maintaining safety and confidentiality.
