Job summary

Optima Medical Management Group is seeking a Community Support Supervisor to ensure efficient operations for Health Plan members in a collaborative, growth‑oriented setting. You will supervise CS staff, oversee reporting, and coordinate referrals and housing deposits with health plans and landlords.

Role requires leadership, strong communication, and familiarity with health plan processes. This is a full‑time on‑site position in Los Angeles, CA with competitive benefits and growth opportunities.

Qualifications

  • High school diploma or GED required; Bachelor's degree preferred.
  • Bachelor's in social work or healthcare administration, or at least 4 years of related work experience.
  • Previous supervisory experience in community support or healthcare.
  • Valid CA driver's license, reliable vehicle, and active CA auto insurance.

Responsibilities

  • Generate and analyze daily and weekly reports for member progress.
  • Maintain and update the Master Log daily.
  • Review Ring Central Analytics, productivity logs, and member notes.
  • Oversee staff notes and authorization processes in Alliance.
  • Collaborate with health plans for referrals and authorizations, ensuring compliance.
  • Coordinate completion of Housing Goods Form with health plans and landlords.
  • Ensure quality and consistency in staff assessments and regulatory adherence.
  • Communicate effectively with stakeholders and maintain documentation.
  • Perform other duties as assigned.

Job Summary

As a Community Support Supervisor, you will play a pivotal role in ensuring the efficient operation of Community Support services to our Health Plan members. You will be responsible for providing supervision, managing daily and weekly reports, coordinating with health plans for referrals and authorizations, overseeing staff assessments, and ensuring housing deposit processes are completed accurately and with proper documentation. Your role will involve effective communication with various partners, including health plans, landlords and members.

Supervisory Responsibilities
  • Provide leadership, guidance, and support to CS staff.
Duties/Responsibilities
  • Generate and analyze daily and weekly reports for member progress.
  • Maintain and update the Master Log daily.
  • Review Ring Central Analytics, productivity logs, and member notes.
  • Oversee staff notes and authorization processes in Alliance.
  • Collaborate with health plans for referrals and authorizations, ensuring compliance.
  • Stay informed on health plan policies and procedures.
  • Coordinate completion of Health Plan Housing Goods Form.
  • Ensure quality and consistency in staff assessments.
  • Monitor assessment processes for regulatory adherence.
  • Coordinate timely completion of housing deposit processes, verifying accuracy with landlords.
  • Communicate effectively with stakeholders, maintaining accurate documentation and promptly addressing inquiries and requests.
  • Performs other duties as assigned.
Education and Experience
  • High school diploma or GED required. Bachelor’s degree preferred.
  • Bachelor's degree in social work or healthcare administration, or at least 4 years of related work experience.
  • Previous experience in a supervisory role, preferably in community support services or healthcare.
  • A valid Driver's License, reliable daily vehicle and active CA auto insurance are required.
Required Skills/Abilities
  • Bi-lingual (English/Spanish) Preferred.
  • Strong leadership and communication skills.
  • Knowledge of health plan requirements and authorization processes.
  • Experience with data analysis and reporting.
  • Ability to multitask and prioritize responsibilities effectively.
  • Attention to detail and accuracy in documentation and record‑keeping.
Physical Requirements
  • Prolonged periods sitting at a desk and working on a computer.
  • Must be able to lift up to 10 pounds at times.
  • Must successfully complete and maintain current Tuberculosis (TB) testing and clearance in accordance with company policy and applicable local/state health regulations as a condition of employment.

Pay range: $29 USD - $32.50 USD
