Job summary

Canopy Children’s Solutions in Mississippi seeks a Community Support Specialist to deliver coordinated community services for children and families. You will assess needs, link families to education, medical, social and recreational resources, and provide crisis support as needed.

You’ll work across home, school and community settings, documenting progress, coordinating transportation, and ensuring safety and compliance with agency guidelines.

Qualifications

  • Bachelor’s degree in social work or related field.
  • At least two years of direct experience with children and families.
  • Certification through the Mississippi Department of Mental Health as a Community Support Specialist.
  • Competence implementing evidence-based practices.
  • Ability to complete reports and payment documentation.
  • Computer skills required.
  • Must be 21+ with valid MS driver license, clean record, and auto liability insurance.

Responsibilities

  • Assists children and families to gain access to education, medical, social, vocational, recreational and other services.
  • Assess strengths and service needs of the children and families.
  • Develop measurable, individualized interventions and track progress.
  • Provide transportation for families in a safe and responsible manner.
  • Direct services in home, school and community.
  • Coordinate social services that are child-focused and build on family/community strengths.
  • Provide crisis intervention as needed within role/competency.
  • Maintain a comprehensive, up-to-date community support resource manual.
  • Attend weekly staff meetings.
  • Collaborate with supervisor, therapist and Canopy staff to monitor progress.
  • Assist with aftercare planning to ensure referrals and appointments.
  • Sits in on telehealth/telepsychiatry as scheduled.
  • Schedule medical appointments for families.
  • Relay changes in medical information to PMHC nurse.
  • Ensure releases for psychotropic medications are signed.
  • Other duties as assigned.

Position Overview
  • The Community Support Specialist (CSS) is a vital care service based on the individual needs of children and their families.
  • The CSS is responsible for delivering an array of accommodating community services, assessing and linking children and families with social, educational, vocational, and recreational services.
  • Collaboratively working with children and families, the CSS provides precise, measurable and individual services that focuses on the child’s and family’s abilities to assimilate within their community as well as to identify and access community services.
  • Since the CSS is an integral resource for children, families and their communities and because it reflects Canopy Children’s Solutions core values, the work functions are comprehensive and include, but are not limited to, identifying and accessing needed community services, implementing evident-based practices specific to individual children and families, connecting them within their communities and tracking their progress.
  • This role in the PMHC also supports the clinic by answering incoming calls; greets and directs visitors who enter the CBHS Outpatient Clinic; takes and processes payments from clients. Additional responsibilities include assisting with the verification of benefits for new clients, scheduling follow-up appointments at client check‑out, supporting the Clinic Manager, and Outpatient Clinic staff as needed.
Required Physical Activities

sitting, standing, walking, reaching, pushing, pulling, grasping, lifting, talking, seeing, hearing, kneeling, stooping, bending, reaching overhead, and climbing stairs as needed and repetitive motions. Must have the physical ability to assist in the physical or therapeutic hold of youth when required, following organization approved safety protocols.

Characteristics of Work

frequent standing and walking, occasional sitting, lifting and carrying objects up to 50 pounds. Must be able to respond quickly to emergencies and physically assist or support youth and/or staff.

Close vision, distance vision, color vision, peripheral vision, depth perception and the ability to adjust focus.

Environmental Conditions

This job requires exposure to both inside and outside weather conditions; the typical noise level for the work environment is moderate to loud noise.

Job Responsibilities
Community Supports
  • Assists children and families to gain access to needed community support services including, but not limited to, education, medical, social, vocational, recreational, therapeutic and other social services.
  • Assess the strengths and service needs of the children and families.
  • Develop specific, measurable, individualized interventions and services to meet needs and measure progress.

Professional Standards
  • Implement service interventions based on the ISP and documents in CareLogic.
  • Implement evidence-based practices and interventions when possible.
  • Complete necessary documentation for each child and family based on program guidelines including CSS notes and support implementation plans in a timely manner.
  • Demonstrate flexibility and work with the family based on the family’s schedule.
Administrative Support
  • Attends weekly staff meetings.
  • Works in collaboration with supervisor, therapist, and other Canopy Children’s Solutions staff to monitor progress.
  • Assist with aftercare planning to ensure referrals are made, appointments are made, and services are available/accessible.
  • Sits in on telehealth/telepsychiatry during scheduled times.
  • Schedules medical appointments for families when needed.
  • Relays changes in medical information including medicine changes to the PMHC nurse.
  • Assures any new psychotropic medications have a release signed by the guardian consenting to the medication prescribed.
  • Other duties as assigned.
Required Qualifications
  • Minimum of Bachelor’s degree in social work or related field and at least two (2) years of experience working directly with children and families.
  • Must meet the minimum requirements to be certified through the Mississippi Department of Mental Health as a Community Support Specialist.
  • Must demonstrate competence implementing evident-based practices, best practices working with children and families.
  • Must demonstrate competence in completing reports related to payment for services provided.
  • Computer skills required.
  • Must be at least 21 years of age, have a valid Mississippi driver license, a good driving record, proof of legally appropriate auto liability insurance, and must be willing to use personal vehicle, as needed, for agency business
