Position Overview

As an Outpatient Clinic Community Support Specialist, you will deliver a range of community services and assist children and families in accessing social, educational, vocational, and recreational resources.

Job Responsibilities

Community Supports

• Assist children and families to gain access to needed community support services including education, medical, social, vocational, recreational, and therapeutic services.
• Assess strengths and service needs of children and families.
• Develop specific, measurable, individualized interventions and services to meet needs and track progress.
• Provide and coordinate child‑focused social services that build on family and community strengths.
• Provide crisis intervention services as required within scope of role and competency.
• Develop and maintain an up‑to‑date community support resource manual.
• Facilitate the intake process, ensuring smooth operation, clear caregiver consents, scheduled follow‑up appointments, and a hopeful outcome.

Professional Standards

• Implement service interventions based on the ISP and document in CareLogic.
• Apply evidence‑based practices and interventions when possible.
• Complete necessary documentation for each child and family, including CSS notes and support implementation plans, in a timely manner.
• Demonstrate flexibility and work with families based on their schedules, including possible evenings and weekends.

Administrative Support

• Attend monthly staff meetings.
• Collaborate with supervisor, therapist, and other staff to monitor progress.
• Assist with after‑care planning: ensure referrals, appointments, and service availability.
• Schedule medical appointments for families when needed.
• Relay changes in medical information, including medication changes, to the PMHC nurse practitioner.
• Ensure any new psychotropic medications have a guardian‑signed release.
• Cross‑train in front desk duties and provide front desk coverage as needed.
• Perform other assigned duties.

Required Qualifications

• Minimum of a bachelor’s degree in social work or related field and at least two (2) years of experience working directly with children and families.
• Meet the minimum requirements for certification with the Mississippi Department of Mental Health as a Community Support Specialist.
• Demonstrate competence in implementing evidence‑based practices and best practices with children and families.
• Demonstrate competence in completing reports related to payment for services provided.
• Computer skills required.
• Availability for a flexible schedule to meet job responsibilities.
• At least 21 years of age, valid Mississippi driver’s license, good driving record, proof of legally appropriate auto liability insurance, and willingness to use a personal vehicle for agency business.
