Qualifications

Responsibilities

• Provide community-based mental health services to adults, including assessment, diagnosis, treatment planning, and intervention implementation.
• Collaborate with clients, families, and stakeholders to develop individualized treatment plans.
• Utilize strengths-based approach with Motivational Interviewing and Trauma-Informed Strategies.
• Coordinate care with primary care physicians and community organizations.
• Maintain accurate and timely records of client progress per policies.
• Participate in staff development activities to enhance knowledge in mental health services.
• Work with team members to ensure coordinated care and client support.
• Prepare for and facilitate ITCD group sessions per treatment plan goals.
• Assist with transportation to community resources and physician appointments.
