Qualifications

• Ability to maintain effective working relationships with diverse individuals and groups.

Responsibilities

• Develop and coordinate services within the community.
• Assess mental health, co-occurring conditions, and functional needs.
• Utilize motivational interviewing during assessment and planning.
• Assist clients to obtain and maintain housing.
• Plan and deliver training on personal, community living, and social skills.
• Assist in transition planning, discharge, and follow-up services.

Education

Bachelor’s Degree in a Human Services-related field

Two years post High School coursework in a Human Services field, plus two years relevant experience
