Brightli seeks a compassionate Community Support Specialist (Disease Management) in Springfield, MO, to support adults and those with SMI/SUD. The role combines mentorship with clinical coordination to improve care and outcomes in our Disease Management program.

Qualifications

  • Bachelor’s degree in Psychology, Social Work, Sociology, Education, Criminal Justice, Family Studies, Counseling, Recreational Therapy, Human Services, Human Development, Child Development, Gerontology, Behavioral Science, Rehabilitation Counseling or related field.
  • Associate of Applied Science in Behavioral Health Support from an approved institution or equivalent.
  • Four years of qualifying experience may substitute for education.

Responsibilities

  • Deliver life-changing mental health services to clients in the community.
  • Empower clients through behavioral interventions, using motivational interviewing and cognitive processing.
  • Foster recovery and resilience by promoting improved functioning and meeting client needs.
  • Manage a caseload and collaborate with supervisory staff to ensure a fulfilling workload.
  • Respond to emergencies with sound judgment and provide timely assistance during crises.
  • Ensure continuity of care by providing comprehensive follow-up services.

Job Summary: We’re seeking a compassionate and driven Community Support Specialist to join our team, with a focus on supporting adults in various capacities while also assisting individuals with Serious Mental Illness (SMI) and/or Substance Use Disorder (SUD) through our Disease Management program. This role combines the responsibilities of providing mentorship, guidance, and support to adults in the community with the opportunity to make a meaningful impact on the lives of individuals with complex health needs. Key Responsibilities


  • Deliver life-changing mental health services to clients in the community

  • Empower clients through behavioral interventions, utilizing techniques like motivational interviewing and cognitive processing

  • Foster recovery and resilience by promoting improved functioning and meeting unique client needs

  • Manage a rewarding caseload and collaborate with supervisory staff to ensure a fulfilling workload

  • Respond to emergencies with confidence, using sound judgment and discretion to provide timely assistance during mental health crises

  • Ensure continuity of care by providing comprehensive follow-up services to clients


Disease Management Responsibilities


  • Review and prioritize outreach efforts to individuals identified through the Disease Management Cohort List

  • Engage individuals using motivational interviewing techniques and a person-centered approach

  • Coordinate with the intake department to ensure timely admission into clinically appropriate programs

  • Complete required documentation for the Disease Management program, including reviews and updates to cohort lists and state reporting

  • Collaborate with the Healthcare Home (HCH) Department to ensure all Disease Management individuals are enrolled in HCH


Qualifications


  • Bachelor’s degree in one of the following fields of study: Psychology, Social Work, Sociology, Education, Criminal Justice, Family Studies, Counseling, Recreational Therapy, Human Services, Human Development, Child Development, Gerontology, Behavioral Science, Rehabilitation Counseling OR...

  • Associate of Applied Science in Behavioral Health Support degree from an approved institution OR....

  • Work experience in a related human services field may substitute for educational requirements. *Related fields may include direct support such as case management, residential support, developmentally disabled support, vocational rehabilitation, and more. Four years of qualifying experience minimum or a combination of education and experience equaling four years required.

  • Qualified Addictions Professional (QAP) - CADC, CRADC, CRAADC, CCJP, RADC-P, RADC, CCDP, CCDP-D
