Job summary

Socket.dev is seeking a qualified professional to manage Community Support Services, maintaining CSS certification and handling annual trainings. You will plan schedules, coordinate with the CSS Director, and provide case management and supportive counseling to clients and families.

The role requires a Bachelor’s degree in social work, counseling, or psychology, strong driving record, and ability to liaise with multiple agencies to secure benefits, housing, and medications for clients.

Qualifications

• Bachelor’s degree in social work, counseling, psychology, or a related field.
• Ability to relate effectively to severely mentally ill individuals.
• Have transportation, hold a valid driver’s license, automobile insurance, and a safe driving record
• Must have, and maintain, a Communicare insurable driving record

Responsibilities

• Obtain and maintain proper certification to perform CSS, taking ownership of this process and completing annual training.
• Meet productivity by planning ahead, scheduling, and seeking support when deficiencies are anticipated.
• Provide case management services for clients who meet criteria for Community Support Services.
• Provide ongoing assessment of clients’ needs and develop appropriate treatment planning.
• Coordinate case management and mental health center services with services from other agencies.
• Provide supportive counseling to clients and families as needed.
• Assist CSS clients in applying for benefits, housing, medications, and medication management plans.
• Ensure access to mental health and other community services for CSS clients.
• Maintain documentation, complete reports, and participate in meetings and trainings.
