Job description

The Community Support Specialist provides integrated, holistic, community-based support services.

ESSENTIAL FUNCTIONS :

Clinical Service Provision

  • Using a motivational, stage-matched approach, provide individualized and group education services based on strengths, needs, abilities, and preferences identified collaboratively with clients/families, focused on supporting clients’ movement toward recovery/resiliency goals.
  • Assist clients in identifying strengths, needs, abilities, and preferences regarding substance abuse/mental illness and participate in the development of Wellness Plans.
  • Provide services from a holistic perspective, incorporating coaching surrounding the Eight Dimensions of Wellness, with attention to each domain of wellness.
  • Support clients in completing goals identified on Wellness Plans, reviewing the plans with clients as needed for updates or changes.
  • Aid clients in accessing public services, including financial, housing, emergency assistance, food, etc.
  • Maintain contact with clients who are hospitalized, participate in discharge planning, and communicate with involved providers for aftercare plans.
  • Provide education and skill-building in areas such as daily living skills, employment/education, symptom management, communication/relationships, budgeting, maintaining a residence, etc., with a focus on moving toward independent completion or referral to long-term supports.
  • Assist clients in developing personal support systems, including working with family members, legal guardians, recovery communities, etc.
  • Provide individual assistance in accessing needed services, including accompanying clients to appointments.
  • Follow up with clients regarding appointments, forms, receipts, and similar activities.
  • Contact clients with unexcused absences from the program to re-engage and promote recovery efforts.
  • Meet service provision expectations based on client needs and company guidelines.
  • Complete documentation and paperwork within required timeframes.

Integrated Health Team

  • Communicate actively with other team members regarding all areas of client wellness, including medication adherence, side effects, and efficacy.
  • Seek and include input from team providers on Wellness Plans.
  • Work with clients to determine needed services and facilitate hand-offs.
  • Make referrals to additional services/providers as needed.
  • Function as a liaison to others involved in the client’s treatment, including family, employers, schools, probation/parole, courts, etc.
  • Seek culturally competent services and providers to meet clients’ cultural needs.
  • Assist clients with transportation as indicated; driving is an essential function of this position.
  • Other duties as assigned.
