Job summary

Start Corporation in Lafayette, LA seeks a Community Support Specialist to provide CPST and PSR services to adults with complex disabilities, helping them achieve self-identified goals.

Responsibilities include case management, developing service plans, coordinating with community resources, and documenting client progress in required systems. A bachelor’s or master’s in sociology, psychology, social work, or counseling is required, along with a valid driver’s license and being 18+.

  • Provides case management and supportive services (social skills, life skills, benefits acquisition, community resource utilization, job search and placement, coping skills, money management) for clients and coordinates services from community resources based on the case plan.
  • Participates in setting case plan goals/objectives.
  • Completes case plans with clients, documents on all client services including data entry into required data systems.
  • Ensures that clinical documentation and client files meet applicable standards.

Job Description

The Permanent Supportive Housing (PSH) program links adults ages 18 and up to affordable rental housing with voluntary, flexible and individualized services to people with severe and complex disabilities, enabling them to live successfully in the community. Community Support Specialist provide CPST (Community Psychiatric Supports and Treatment) and PSR (Psychosocial Rehabilitation) services to assist participants in achieving self-identifying goals.

Minimum Requirements
  • Bachelor or Master's Degree in Sociology, Psychology, Social Work or Counseling
  • Be over 18 years of age
  • Possess a valid drivers license

Job Duties / Skills Required
