Qualifications

Responsibilities

• Assess consumer's overall functioning, identifying problems, needs, skill strengths and deficits, and lead the consumer-centered planning process and develop implementation strategies.
• Implement selected aspects of the plan including skill building, counseling, advocacy, and building community, family, and natural supports.
• Ensure linkages to community service providers, entitlement programs and housing alternatives.
• Advocate for individual's basic need, development of natural resources, and personal independence through teaching self-advocacy skills.
