JOB SUMMARY:
Provides community case support for participants active in an accountability court program. Works directly with individuals diagnosed with substance use and/or mental health disorders involved in the criminal justice system. Responsibilities include home visits, transportation assistance, vocational support, ensuring attendance at treatment and court sessions, and conducting medication counts. Reports to the Program Coordinator.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
MINIMUM QUALIFICATIONS REQUIRED:
One year full-time (or equivalent part‑time) experience in social services delivery (case management, outreach, prevention/education, etc.) OR one year full‑time experience performing direct care activities, basic nursing procedures, or client training at the level of a health services technician.
Associate’s degree in Social Work, Psychology, Nursing, Education, or related field required.
Must be able to work in a fast‑paced court environment with limited supervision.
Must have working knowledge of Microsoft Office Suite.

Valid Georgia Driver’s License
