Job summary

North Los Angeles County Regional Center (NLACRC) is seeking a Service Coordinator for AV Transition 2/ School Age 4 to provide information, advocacy, and service arrangement for consumers and families.

The role includes developing program plans, coordinating services, documenting in NLACRC records, advocating for individuals, attending interdisciplinary meetings, and supporting team members while meeting regulatory requirements. Flexibility for in-home and community meetings is required.

Qualifications

• Bachelor's degree in psychology, social work, sociology, or related human services field and at least two years of related experience. A Master's degree may substitute for experience.
• Understanding of family systems, person-centered planning, and ability to work in interdisciplinary teams; strong written and verbal communication; familiarity with MS Word and Outlook.
• Bilingual Spanish may be required depending on position.

Responsibilities

• Participation in development of program plans for individuals; monitor and revise plans as needed.
• Identify and coordinate services for individuals and their families.
• Documentation and reporting in accordance with regulations and NLACRC policies.
• Advocate for individuals served by community agencies.
• Schedule and attend interdisciplinary meetings with individuals, families, service providers, and advocates.
• Address barriers to service delivery with empathy, trust, and respect; support in non-traditional settings when needed.
• Rotate on-call duties and provide coverage for emergencies when required.
