Job summary

The City of Lakewood in Lakewood, OH is seeking a full-time Community Resource Specialist in the Human Services Department, Division of Youth. The role serves as the resource and referral source for the Community Based Services Program and offers an hourly rate of $25.29.

Qualifications

Responsibilities

• Provide information, referral and short-term case management to families in need.
• Document and track referral sources and types of referrals.
• Maintain required files for the agency and program.
• Coordinate family team meetings and home visits.
• Support program staff with emergency assistance and home visits.
• Attend required training and team meetings.
• Assist families through eviction processes.
• Assist with planning, coordination and documentation of programming/events.
• Research targeted community resources as needed.
• Attend local/county meetings to build connections.
• Maintain an updated community resource guide.
