Candidate must reside in or be willing to travel throughout Greensboro, NC and surrounding areas, including Charlotte, Raleigh-Durham, and counties such as Rockingham, Guilford, Randolph, Forsyth, Stokes, Yadkin, and Surry. Nearby counties will also be considered.
Position Purpose: Works with care management team on community resource connection activities including connecting members to community resources to support their care management journey and provide necessary care resources in a cost-effective manner. Provides members with known community resources and works with the care team to identify member community support and provide health education as appropriate.
Education/Experience: Requires a High School diploma or GED
Requires 2 - 4 years of related experience
Preferred Qualifications:
Certified Community Health Worker (CHW) certification is preferred but not required.
Experience in housing navigation or housing services is desirable; training will be provided for the right candidate.
Background in case management or related support services.
Comfortable working autonomously in a dynamic and fast-paced environment.
Demonstrated experience engaging with individuals experiencing homelessness, living with disabilities, or from marginalized communities.
Strong interest in community engagement; enthusiasm for working in the field is a significant plus, as this role is highly community-oriented.
Must possess a valid North Carolina driver’s license and have reliable transportation.
