Centene is currently hiring a Community Resource Coordinator III for our Illinois market. The ideal candidate will have experience with home visits, community events, and a background in social services. This position is 80% field-based and covers counties including Randolph, Perry, Washington, Clinton, Bond, Fayette, Jefferson, Marion, Effingham, Jasper, Crawford, Clay, Richland, Lawrence, Wayne, Edwards, and Wabash.
Works with the care management team on community resource connection activities, including connecting members to community resources to support their care management journey and providing necessary care resources in a cost-effective manner. Provides members with known community resources and collaborates with the care team to identify member community support and health education as appropriate.
Requires a High School diploma or GED and 2-4 years of related experience.
Pay Range: $20.00 - $34.03 per hour. Offers comprehensive benefits including health insurance, 401K, stock purchase plans, tuition reimbursement, paid time off, and flexible work arrangements.
