Job summary

Job Title: Community Resource Coordinator

Status: Non-Exempt, Full-Time (40 hours per week)

Pay Rate: $20-$22/hr

Reports to: Program Manager

Job Summary

Under the supervision of the Program Manager, the Community Resource Coordinator is responsible for assisting clients in gaining access to workforce development resources and increasing access to hunger relief and healthcare services for underserved populations in San Bernardino and Riverside Counties. This position includes providing support for employment readiness, skills training, CalFresh enrollment, nutrition education, healthcare outreach and enrollment services such as Medi-Cal, Covered California, and Enhanced Care Management (ECM).

The Community Resource Coordinator works collaboratively with internal teams and external partners to empower clients through education, enrollment assistance, and direct services.

Essential Duties and Responsibilities

To successfully fulfill this role, the individual must perform the following duties to a satisfactory standard:

Case Management
  • Conduct comprehensive client intake and needs assessments to develop individualized service plans, coordinating all necessary referrals and support resources.
  • Manage and maintain accurate client schedules, including booking appointments, arranging follow‑up visits, sending timely reminders, and resolving any calendar conflicts to ensure consistent service delivery.
  • Document all case activities and communications in the case management system, track client progress against goals, and collaborate with interdisciplinary teams to adjust scheduling and care plans as needed.
Program Administration
  • Track and report program outcomes, including client employment outcomes, enrollment numbers, distribution data, and event participation for the Program Manager and other stakeholders as needed
  • Ensure compliance with local, state, and federal regulations related to employment, hunger, and healthcare programs
  • Ensure compliance to meet contractual responsibilities of career advisory, hunger, and healthcare programs
  • Contribute to strategic planning and continuous improvement initiatives
Client Support Services
  • Assist clients in identifying and applying for employment opportunities that align with their skills, certifications, and career goals.
  • Assist clients with CalFresh, Medi-Cal, and Covered California applications and recertifications, ensuring applications are complete and accurate
  • Provide individualized support to address eligibility and documentation for each program
  • Follow up with clients to ensure successful enrollment and continued access to services
Community Education and Outreach
  • Host community workshops and events to promote awareness of workforce development programs, food security and healthcare access
  • Develop and distribute educational materials and outreach campaigns
  • Foster a welcoming and supportive environment for clients and volunteers.
Partnership Development
  • Maintain collaborative relationships with businesses and organizations to identify job openings and workforce development resources
  • Maintain collaborative relationships with food banks, healthcare providers, local organizations, and volunteers to expand service reach
  • Represent Catholic Charities at community events and building strong relationship with stakeholders
Community Gardens
  • Coordinate community garden programs to provide fresh, locally grown produce to underserved populations.
  • Collaborate with local organizations, volunteers, and community members to establish and maintain garden spaces.
  • Offer workshops and training on gardening techniques, nutrition, and sustainability.
Food Distribution
  • Organize and oversee food distribution events to ensure equitable access to nutritious food.
  • Collaborate with local food banks, community organizations, and volunteers to manage logistics and inventory.
  • Track distribution data to measure impact and identify areas for improvement.
Other Duties as Assigned
  • Perform additional tasks as needed to support the overall goals and mission of Catholic Charities.
  • Available to work at any Inland Empire location or remotely, as directed by the Supervisor.
Qualifications
Education:
  • Bachelor’s degree in Social Work, Public Health, or a related field preferred.
  • Equivalent work experience in workforce development, hunger relief, healthcare enrollment, social services, or community outreach may be considered.
  • Upon hire, employees will be expected to participate in professional development opportunities, which may include obtaining a Community Health Worker (CHW) Certification, completing Career Advisory courses, and participating in other job-related trainings, certifications, and professional development activities as assigned by the Agency.
Experience:
  • Minimum of two years of experience in workforce development, food assistance programs, healthcare enrollment, community outreach, or case management.
  • Familiarity with CalFresh, Medi-Cal, and Covered California is preferred
Skills and Abilities:
  • Strong interpersonal and communication skills, including the ability to engage with diverse populations.
  • Excellent organizational skills and attention to detail.
  • Proficiency in Microsoft Office Suite and CRM software.
  • Demonstrated ability to manage multiple priorities and meet deadlines.
  • Ability to work independently and collaboratively in a team-oriented environment
Physical, Mental, and Environmental Requirements
  • Ability to travel throughout San Bernardino and Riverside Counties as needed.
  • Work in diverse settings, including office environments, food distribution events, and community garden, community events, and client homes
  • Occasionally lift and carry items weighing up to 30 pounds.
  • Flexibility to work evenings and weekends as required.
