Job summary

Azza HealthCare Agency is seeking a Licensed Physical Therapist to evaluate functional mobility and rehabilitation needs, and to develop individualized treatment plans aimed at improving independence and community integration. Qualifications

Licensed Physical Therapist with strong assessment and clinical reasoning skills.

Responsibilities

Evaluate functional mobility and related rehabilitation needs.
Develop and implement evidence-based, individualized treatment plans.
Provide direct therapeutic interventions to promote independence and community integration.
Collaborate with interdisciplinary teams to support holistic care.
Document clinical findings and interventions in electronic health record (EHR) systems.
Educate patients and caregivers on care strategies and functional goals.
Track and report functional outcomes to support data-driven care.
