Compass Health, Inc. is seeking a Community Support Specialist to support adults facing behavioral health and substance use challenges at Cedar Recovery Center in Linn Creek.

The role delivers person-centered support across home, community, and office settings, connecting clients to essential resources and coordinating care with an integrated team for holistic wellness. Qualifications

• High School/GED required.
• Associate's/Bachelor's degree in a human service-related field preferred.
• Must qualify for CSS position per CSS requirements (various options listed).
• All higher education degrees must be from accredited schools.
• Qualifying experience includes delivering services to individuals with mental illness, substance use disorders, or developmental disabilities.
• Must have a valid, unrestricted driver's license and auto insurance.

Responsabilités

• Provide individualized and group community support services using a person-centered, recovery-oriented approach.
• Partner with individuals to identify strengths, establish wellness goals, and support progress toward independence.
• Connect clients with healthcare, housing, financial, employment, educational, and other community resources.
• Teach and reinforce skills promoting daily living, wellness, recovery, and long-term stability.
• Coordinate care with interdisciplinary providers, family, and community supports.
• Assist with care transitions, discharge planning, referrals, and ongoing engagement in services.
• Provide transportation and accompany clients to appointments as needed.
• Maintain timely documentation and required screenings.
