Job summary

EverTrue is seeking a Community Outreach & Care Manager to bridge clients, families, referral partners, and the Anywhere Care team. This hybrid role combines clinical social work with healthcare outreach to help older adults navigate care options and build community relationships in the St.

Louis metro area. You will educate families on available EverTrue services, support enrollments, and grow the Anywhere Care program by tracking referrals and outcomes.

Qualifications

  • Bachelor of Social Work (BSW) or Master of Social Work (MSW) required.
  • Experience in healthcare outreach or community outreach.
  • Proven ability to build relationships with healthcare providers, hospitals, physicians, senior living communities, or referral sources.
  • Excellent communication, relationship-building, and presentation skills.
  • Ability to travel throughout the St. Louis metro area.

Responsibilities

  • Support Clients & Families: conduct psychosocial assessments and develop individualized care plans.
  • Guide clients and families through care decisions, transitions, and long-term planning.
  • Identify barriers to care and connect clients with resources.
  • Educate families on EverTrue Home Help, At Home+, GUIDE, and other services.
  • Manage new client enrollments and execute service agreements.
  • Develop and maintain relationships with hospitals, discharge planners, physicians, clinics, and senior living communities.
  • Represent EverTrue to network events and community meetings.
  • Facilitate handoffs between referral partners and intake teams.

Every day, our team helps older adults and their families navigate important care decisions with compassion, expertise, and respect.As a Community Outreach & Care Manager, you'll serve as the bridge between clients, families, referral partners, and the EverTrue Anywhere Care team.
