Baptist Health Lexington and Richmond are seeking a Community Staff Pharmacist to support the Community Pharmacy Manager in directing pharmacy staff and services. You will provide direct patient care, ensure dispensing accuracy, immunization administration, and oversee inventory including 340B program integrity.
Responsibilities include staff development, policy compliance, reconciliation, and safe transitions of care.
This position assists the Community Pharmacy Manager with the direction and development of pharmacy staff to optimize patient experience and delivery of services. The Community Staff Pharmacist provides direct patient care services including but not limited to drug regimen review, disease state management, medication counseling, adherence monitoring, discharge medication reconciliation, prescription dispensing, and immunization administration. Proficient with all steps of dispensing and verification workflows. This position is responsible for performing all pharmacy operations in accordance with accredited policies and procedures, ethical and professional practices, Board of Pharmacy, and State and Federal requirements. The Community Staff Pharmacist maintains appropriate patient record documentation for clinical and dispensing services. Adheres to best practices for medication safety and ensures appropriate documentation of medication events. The Community Staff Pharmacist is responsible for inventory management functions, including maintaining 340B program integrity as applicable to practice site.
