Job summary

WellSpan Health seeks a traveling pharmacist to support six pharmacy sites across our network. The role performs compounding, patient counseling, and drug information services to ensure safe and effective medication use.

Responsibilities include monitoring therapy, resolving drug-related problems with clinicians, and providing immunization services as part of patient care. A Doctor of Pharmacy degree and active license are required.

Qualifications

• Licensed pharmacist with Doctor of Pharmacy degree or equivalent.
• Completion of degree program from accredited school of pharmacy.

• Compounds and labels medications; supervises IV preparation as needed.
• Advises patients and medical staff on drug use, interactions, and allergies.
• Ensures proper controls and records for narcotics and other controlled substances.
• Monitors drug therapy and collaborates with physicians and nurses to resolve issues.

Full-Time: 40 Hours/Weekly

Varied days, evenings, potential weekend rotation

Floating pharmacist position that will travel among 6 different pharmacy sites.

General Summary

Provides pharmaceuticals and drug information as needed for adequate patient care according to professional standards and practices.

Responsibilities

Essential Functions

• Compounds pharmaceutical preparations and/or labels and packages medications and pharmaceuticals. Assists with and/or supervises IV preparation and/or delivery.
• Advises patients and medical/nursing staff on use of various medications including dosage, side effects, composition, drug-to-drug interactions, drug-to-allergy interactions and drug-to-food interactions.
• Ensures proper controls and records of narcotics and other controlled substances as required by state and federal law.
• Monitors drug therapy to identify, prevent or resolve actual or potential drug therapy problems. Addresses problems with physicians and/or nurses when indicated.
• Provides clinical pharmacy services as needed.
• Administers immunizations, reviews patient profiles for immunization qualification and makes that recommendation to the patient.

• Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment. Provides direct patient care to assigned patient age group(s).
• Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause analyses as required.
• Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards.
• Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.
• Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.
• Assists with and/or supervises personal care/long term care medication delivery.
• Assists in preparing required reports by compiling information.

Qualifications

Minimum Education

• Bachelors Degree Pharmacy Bachelor's Degree Required

Work Experience

• Less than 1 year Relevant experience. Required

Licenses

• Licensed Registered Pharmacist Upon Hire Required and
• Authorization to Administer Injectables for Pharmacist Upon Hire Required and
• Basic Life Support Upon Hire Required

Courses And Training

• Completion of degree program from accredited school of pharmacy. Upon Hire Required
