Job summary

Required CPS-MH certification, at least 6 months CPS experience, a high school diploma or GED, and proficiency with electronic medical records.

Qualifications

• CPS-MH certification required.
• 6 months CPS experience in adult mental health preferred.
• Ability to engage individuals in treatment and self-advocacy.
• High School diploma or GED.
• Proficient computer skills to use EMRs.

Responsibilities

• Prepares and provides recovery-oriented psycho-educational groups.
• Manages a caseload with care coordination and referrals.
• Assists in evaluating progress and developing individual recovery plans.
• Educates and advocates with family members and other caregivers.
• Documents contacts and daily progress notes.
• Uses EMR daily and coordinates with site staff.
• May transport participants to community integration outings.
• Occasional weekend or adjusted hours may be required.
