Job summary

CareCollab seeks an Account Manager – Community Partnerships & Referrals to build and maintain relationships with referral sources across NYC. You will educate partners about programs, eligibility, and referral processes, while traveling to meet partners and attend events.

The role emphasizes relationship-building, field presence, and strong communication to grow qualified referrals and ensure partner success in CareCollab’s programs.

Qualifications

• 3+ years of experience in account management or related field.
• Experience developing and maintaining professional relationships with external organizations.
• Healthcare, behavioral health, social services, Medicaid, care management, housing or related env. preferred.
• Strong understanding of NYC healthcare or social-service landscape preferred.
• Excellent verbal and written communication skills.
• Strong interpersonal skills and ability to build trust at all levels.
• Comfortable conducting presentations, meetings, and outreach independently.
• Highly organized with strong follow-through and attention to detail.
• Ability to manage multiple relationships and priorities simultaneously.
• Comfortable in the field and traveling throughout NYC.
• Proficiency with CRM systems, Microsoft Office/Google Workspace, and other business tech.
• Must be comfortable in a fast-paced, growing organization where priorities evolve.

Responsibilities

• Develop & maintain community partnerships with healthcare and social-service organizations.
• Serve as trusted contact for referral partners and explain CareCollab services and eligibility.
• Conduct regular in-person visits, meetings, and outreach across NYC.
• Identify opportunities to deepen relationships and expand CareCollab’s presence.
• Represent CareCollab at events, conferences, and networking opportunities.
• Educate partners on programs, referrals, and value proposition; follow up on barriers.
• Monitor referral activity and improve partner engagement with internal teams.
• Create strategic account plans and maintain consistent partner touchpoints.
• Track activities in CRM and ensure timely partner communications.
• Identify cross-organizational opportunities and drive collaboration.
