Job summary

Community Medical Services (CMS) is hiring a Community Impact Specialist to support patients recovering from addictions and foster relationships with community partners across clinics, hospitals, and EMS services.

You will coordinate care between opioid treatment programs and community agencies, helping CMS staff provide a safe path to recovery. Travel regionally is required; bilingual helpful.

Qualifications

• Experience in substance abuse rehabilitation and/or opioid treatment programs preferred.
• Bilingual ability preferred.
• Ability to travel within the region and between CMS locations.
• Valid Driver License and motor vehicle clearance required.

Responsibilities

• Identify and access community resources to support clients.
• Provide timely documentation of client and agency encounters, and coordination activities.
• Maintain program data in multiple databases (QuickBase, Methasoft, CRM).
• Coordinate care with CMS clinics and community system partners.
• Attend provider and community meetings to advocate and build relationships.
• Support program development with project management and referrals processing.
• Process referrals from CMS clinics and community partners within 24 hours.
