Job Description:
The Community Partner Liaison (CPL) will create and maintain trusting and collaborative relationships with community partnerships that serve (0-5 partners, schools, community organizations) and community partner administration and staff. Community Partner Liaisons (CPL) hold monthly meetings with each partnership to discuss referral process, agency updates and “temperature check” meetings to ensure partnerships are operating smoothly. Community Partner Liaison is also tasked with the maintenance and processing of clinical referrals sent to the organization through these partnerships. The Community Partner Liaison serves as the primary point of contact at Valle Del Sol, facilitating connections between patients, families, and necessary services. This role involves processing new referrals from schools and community partners, ensuring patients are promptly scheduled to begin behavioral health services, and performing outreach to community partnerships through activities such as tabling, marketing events, and school events.
