Job summary

EMS Unlimited seeks a Community Paramedic to launch a Mobile Integrated Health program in Garfield County. You’ll see patients in homes, coordinate with physicians, and operate under CP protocols with telehealth access to supervisors.

You’ll perform in-home assessments, manage chronic conditions, and coordinate post-discharge care while adhering to Colorado CP standards and safety requirements. This is a full-time role with comprehensive benefits.

Qualifications

• Colorado Community Paramedic endorsement completed within 2 months of hire.
• Current CPR/BLS and ACLS.
• Minimum 2 years of paramedic-level field experience.
• Proficient in 12-lead ECG interpretation, IV access, and venipuncture.
• FEMA ICS-200 and ICS-700.
• Valid driver’s license with a clean record.

Responsibilities

• Complete a full in-home history and physical exam, including vital signs, ECG, and monitoring.
• Perform point-of-care testing (i‑STAT, glucose) and venipuncture as ordered.
• Use point-of-care ultrasound within protocol and training.
• Provide wound care, IV access, and medication administration.
• Recognize decline early, telehealth consult, and arrange the appropriate level of care.
• Make post-discharge home visits within 24–72 hours of discharge.
• Coordinate care and document findings for primary care providers and care transitions.
