Job summary

Allina Health is seeking a Nurse Practitioner to join the Community Palliative Care and Hospice team in Minneapolis. The role delivers patient-centered care in both inpatient and outpatient settings, primarily in palliative care with a hospice component.

The position requires a Master’s in Nursing (NP), MN RN licensure, ANCC certification, ACLS/BLS, and a valid driver’s license. Day shift, 8-hour shifts, 32 hours/week, full-time employment.

Qualifications

• Master's degree in Nursing (NP) from an accredited program.
• Licensed RN in MN and ANCC CNP certification within specialty on hire.

Responsibilities

• Provides clinical care as part of a physician/nurse team in inpatient and outpatient settings.
• Performs comprehensive patient assessments, diagnoses and treatment planning.
• Orders and interprets diagnostics, prescribes medications and therapies.
• Educates patients and refers to specialists as needed.
