$90,000 Salary + Quarterly Performance Incentives (Up to $20K Annually)

About the Role

The Community Liaison is a vital connector between our healthcare services and the community. This role drives business development, outreach, and relationship-building with referral partners, patients, caregivers, and the broader community.

What You’ll Do

• Build and maintain strong relationships with referral sources (hospitals, physician offices, senior living communities, healthcare providers).
• Identify new business opportunities to grow patient census across service lines.
• Represent the Hospice at community events, health fairs, and networking functions.

Patient & Family Engagement

• Educate patients, families, and caregivers on hospice services and eligibility.
• Serve as a compassionate resource to guide families through the enrollment process.

Communication & Coordination

• Facilitate smooth communication between referral sources and clinical teams.
• Ensure timely follow-up on referrals and accurate documentation of outreach.

Market Leadership

• Act as the face of the Hospice in your community, building trust and awareness.
• Monitor market trends and competitor activity to inform strategy.

What We’re Looking For

• Proven experience in healthcare marketing, sales, or community outreach (hospice, home health, or related field preferred).
• Strong track record of meeting/exceeding referral and census growth goals.
• Excellent interpersonal and communication skills, able to connect with diverse audiences.
• Knowledge of Medicare/Medicaid, hospice eligibility, and CRM systems a plus.
• Ability to work independently and collaboratively in a fast-paced environment.
• Valid driver’s license and reliable transportation required.
