Community Outreach Representative
The Community Outreach Representative is responsible for engaging Medicare seniors to create organic patient growth in local communities. The role works closely with local community partners and clinics to ensure seniors have access to value-based preventive primary care. This position reports to the local Outreach Community Manager.
Responsibilities
- Deliver and coordinate educational presentations to existing and prospective patients.
- Create and maintain working relationships with referral sources.
- Attend lead‑generating events in the community and at our clinics.
- Engage potential members through phone, text, and email.
- Manage and report on leads through CRM or other systems.
- Collaborate with health plan partners for coordination.
- Develop and maintain local broker relationships.
- Coordinate with marketing to promote events.
- Conduct prospective patient tours in partnership with operations.
- Develop relationships with other community partners, such as non‑profits, to acquire new patients.
- Collaborate with operations and clinic staff to coordinate services/events for prospective patients.
- Collaborate with the broker and local health‑plan community to increase awareness and patient enrollment.
- Collaborate with marketing to engage in social media and content development.
- Hybrid schedule (50% field / 50% home office).
- Convert prospective patients to enrolled/active patients with booked visits and assigned PCP.
Qualifications
- Bachelor’s degree in business, sales, marketing, or equivalent work experience.
- At least two years of experience working in the healthcare sector.
- At least one year with Medicare or experience with Value‑Based Care is a plus.
- Experience working in a quota‑based compensation model.
- Proficiency with PowerPoint and strong presentation skills.
- Experience with reporting and tracking through Excel.
- Excellent written and verbal communication skills.
Salary Range
Base salary: $60,000–$65,000 plus incentive compensation (total earnings up to $120,000 based on performance).
