About the Role:
Sweetwater Care LLC is seeking a highly motivated and knowledgeable Outreach & Recruitment Coordinator to drive the growth and continuity of our provider and direct care worker (DCW) network. This is a mission-critical position designed to support Arizona families through upcoming shifts in parent provider eligibility and to expand Sweetwater’s footprint in the ALTCS, private-pay, and non-parent provider spaces.
The ideal candidate brings hands-on experience navigating AZ DDD, DES, ALTCS, and associated systems, along with a deep understanding of compliance, credentialing, and service continuity. You will serve as a connector between families, DCWs, service coordinators, and referral partners—helping us grow the right way, for the right reasons.
Key Responsibilities:
DCW & Family Recruitment (Primary Focus):
Cold call AZ DDD service coordinators and families to present Sweetwater Care as a preferred provider
Actively recruit and onboard non-parent DCWs, especially in response to upcoming state hourly caps (40 hrs/week July 1; 20 hrs/week Oct 1)
Support existing Sweetwater DCWs transitioning to new clients to preserve hours and continuity of care
Explain credentialing requirements and assist with onboarding: CPR/First Aid, Article 9, Fingerprint Clearance, Background Checks
Community & Partnership Development:
Drop-in visits and outreach to strategic referral sources including:
Day programs, clinics, and therapy centers
Vocational schools and CNA training programs
Cultural centers, churches, and senior services
Build referral pipelines with organizations that regularly interact with potential DCWs or families in need of care coordination
System Navigation & Compliance Support:
Guide recruits through 3rd-party systems including FOCUS, Vichra/VTrack, and EVV platforms
Ensure new hires and families are properly documented, credentialed, and prepared for onboarding
Understand service authorizations, reimbursement codes, and EVV regulations
Private Pay & ALTCS Growth Initiatives:
Identify and engage families using private insurance or cash reimbursement
Help establish pricing and communication models for supplemental service offerings
Support development of a care team pipeline for senior citizens and privately funded members through ALTCS and beyond
Qualifications:
Minimum 1–2 years experience in AZ DDD/DES, ALTCS, or HCBS agency work Familiar with service delivery, credentialing, and compliance documentation Proficient in navigating Vichra/VTrack, FOCUS, or similar systems Experience cold calling or community canvassing preferred Comfortable with flexible field outreach and occasional in-person visits Bonus: Knowledge of CNA programs, senior care programs, and vocational pipeline resources Bonus: Familiarity with private pay and ALTCS billing models
