The role serves as a liaison with community partners, stakeholders, and prospective participants, promoting MHFV’s H.O.P.E program through relationship-building, education, and strategic engagement while aligning with funding requirements and organizational goals.

Qualifications

• Bachelor’s degree in human services, public health, communications, marketing, or related field.
• Minimum of two years in outreach, community engagement, or related roles.
• Strong interpersonal, communication, and presentation skills.
• Ability to work independently and manage time effectively in a community-based setting.
• Valid driver’s license and ability to travel within the service area.

Responsibilities

• Serve as a visible representative of MHFV and the H.O.P.E. program.
• Conduct outreach activities in Region 1 and surrounding areas (shelters, clinics, hospitals, courts, DFCS offices, faith-based organizations, community events).
• Educate partners, stakeholders, and the public on MHFV programs and recovery resources.
• Distribute educational materials and promote peer-led recovery services and harm reduction.
• Develop and maintain partnerships with community agencies and referral sources.
• Identify resources to support participant needs (housing, treatment, healthcare, employment, childcare, transportation, legal services).
• Establish bi-directional referral pathways for continuity of care.
• Maintain a community resource directory for staff and participants.
• Develop and implement outreach strategies; build relationships with referral sources.
• Coordinate with internal program staff to ensure timely follow-up on referrals.
• Track referral activity and outcomes to assess outreach effectiveness.
• Engage individuals to assess interest, eligibility, and readiness for services.
• Provide basic screening and linkage to programs; assist with intake navigation as needed.
