Job summary

Community Healthcore in Texas is seeking an Outreach Coordinator who will provide Information and Referral to callers and walk-ins across a 14-county region, ensuring access to long-term services and community resources.

Applicants should have a High School diploma with 2 years of outreach experience; a Bachelor's degree is preferred. Strong verbal and written communication skills, public speaking, and a valid Texas driver’s license are required.

Qualifications

• Bachelor’s degree preferred.
• Excellent verbal and written communication skills; strong public speaking ability.

Responsibilities

• Provide Information and Referral (I&R) to callers and walk-ins for a 14-county area.
• Link clients to long-term services and supports and to local resources.
• Enter data into the tracking system and complete LTSS screens as prerequired.
• Maintain and update the regional resource database.
• Assist clients with the Your Texas Benefits portal through the Community Partners Program.
• Represent the ADRC at outreach events across East Texas.
• Demonstrate excellent phone etiquette and customer service.
