Job summary

Hopscotch Primary Care is seeking an Associate Outreach Leader to grow patient access and engagement across clinics. You will build relationships with local organizations, coordinate outreach events, and guide prospective patients through enrollment.

You will partner with outreach leaders, clinic teams, marketing, and patient engagement to maximize community presence and drive patient growth. This role emphasizes seniors engagement and accessible healthcare navigation.

Qualifications

• High school diploma or equivalent, valid driver’s license, U.S. work authorization.
• Ability to travel locally throughout assigned territory.
• Comfort using Microsoft Office and CRM systems.
• Preferred: 1–3 years of healthcare outreach, community engagement, sales support, event coordination, or customer service experience.
• Preferred: experience with Medicare or senior populations, event planning or community engagement, and CRM/Salesforce experience.

Responsibilities

• Build and maintain relationships with grassroots community organizations.
• Represent Hopscotch Primary Care professionally within the community and serve as a trusted contact for prospective patients.
• Educate community members on Hopscotch services and the value-based care model; conduct outreach activities.
• Maintain recurring touchpoints with community organizations to support long-term engagement.
• Co-lead or support outreach events including health fairs and seminars.
• Support planning and execution of clinic open houses, including venue prep and staffing.
• Greet and engage prospective patients at events and follow up to support conversion.
• Track outreach activities and enrollment outcomes; document in Salesforce.
