Responsibilities

• Establish Upward Health’s presence in the community and educate potential patients about offerings
• Manage a personal caseload with phone outreach, in-person visits, and other community-based strategies
• Enroll patients into Upward Health’s programs and facilitate smooth onboarding
• Collaborate with the Outreach Manager and team to optimize outreach effectiveness
