Job summary

Hackensack Meridian Health is seeking a Community Outreach Coordinator II to plan, coordinate and implement the CHNA and to develop strategies addressing community health needs in support of our mission. The role emphasizes collaboration across teams and engagement with diverse populations.

The ideal candidate will have a Bachelor’s degree, strong organization, and communication skills, plus 5+ years of community-based experience and proficiency in Google Suite/Microsoft Office.

The Community Outreach Coordinator II is responsible for planning, coordinating and implementing Hackensack Meridian Health's (HMH) Community Health Needs Assessment (CHNA) as well as developing strategies to address identified community health needs and in support of Hackensack Meridian Health's strategic imperatives and centers of excellence.

Education, Knowledge, Skills and Abilities Required:

• Bachelor's degree.
• Excellent organization and communication skills.
• 5+ years experience working in the community setting with diverse populations.
• Excellent written and verbal communication skills.
• Proficient computer skills that include but are not limited to Google Suite and/or Microsoft Office platforms.

Education, Knowledge, Skills and Abilities Preferred:

• Master's in public health or related field
• Bi-lingual (Spanish) a plus
• CHES a plus

Licenses and Certifications Required:

• Current Driver's License from a USA state.
