Town of Sherborn, MA seeks a qualified Outreach Coordinator for the Council on Aging. This 24-hour per week, non-exempt position requires a consistent weekly schedule of at least four days during regular hours of operation.

• Bachelor’s degree in social work, gerontology, or human services field, and one to three years of experience with older adults, social work, or human services administrative experience, or an equivalent combination of education, training, and experience.

Responsabilidades

• Provides human services outreach to the Town’s older adults by maintaining regular contact, assessing needs, and offering information, programs, and support services.
• Makes referrals, responds to crises, and coordinates with local agencies and Town departments.
• Collaborates in a team-based environment to ensure continuity of services for residents while maintaining defined responsibilities and accountability.
