Job summary

Liquid Personnel is recruiting for its client an experienced Occupational Therapist to join Hackney Council's Adults, Health & Integration Service, working within the OT and Sensory Service.

This is a fantastic opportunity to work in a busy Local Authority service, delivering person-centred assessments and interventions to promote independence, safety and wellbeing for adults in the community.

Qualifications

• HCPC-registered Occupational Therapist with current registration.
• Experience completing adult community assessments and interventions.
• Strong ability to work within a multidisciplinary team.

Responsibilities

• Complete comprehensive Occupational Therapy and functional assessments for adults with complex needs.
• Assess activities of daily living, mobility, transfers, environmental risks and barriers to independence.
• Recommend and prescribe equipment, adaptations and solutions to support independent living.
• Develop practical, person-centred interventions to maximise independence and safety at home.
• Collaborate with service users, carers, Social Workers, health professionals and multidisciplinary teams.
• Contribute to care planning, risk management and onward referrals.
• Manage a varied caseload prioritising need and risk.
• Maintain accurate assessments, reports and records.
• Apply strengths-based, person-centred practice and relevant health and social care legislation.
