Job summary

NorthShore Health Centers is seeking an Optometrist to examine patients, diagnose eye conditions, and prescribe corrective lenses or treatments. The role requires Indiana licensure and adherence to credentialing guidelines.

You will collaborate with the patient care team, participate in QA processes, and help develop department protocols. The position supports ongoing professional development within a patient-centered PCMH framework.

Qualifications

  • Must have graduated from an accredited optometry program (OD).
  • Must hold or be eligible for Indiana Optometric License and CPR certification.
  • Must maintain credentialing with NorthShore Health Centers and Indiana license requirements.
  • Must hold Valid Indiana Optometric Legislation Drug Certificate.

Responsibilities

  • Examine patients to determine vision problems and eye diseases.
  • Prescribe eyeglasses, contact lenses, and treatments as appropriate.
  • Refer patients to ophthalmologists when needed.
  • Assist in developing optometry department protocols and QA processes.
  • Participate in management meetings and long-range planning.
  • Ensure compliance with applicable regulations and quality standards.
  • Collaborate with clinicians and staff to optimize patient care.
  • Manage administrative duties as assigned by the Director of Optometry.
  • Travel to NorthShore locations only as needed.
