Responsibilities

• The Community Navigator will work with the treatment teams at Youth Behavioral Hospital to provide short-term care coordination and connection to resources for individuals and guardians transitioning from inpatient to outpatient care.
• As the position grows, the Community Navigator will serve as a resource for individuals and families navigating the health care system.
