Job summary

AbsoluteCARE Medical Center & Pharmacy is seeking a Community-based Medical Assistant/EMT supporter to assist the CB-NP with home visits and community care. You will help update discharge plans, perform medication reconciliation, and educate patients, families, and caregivers to promote continuity of care.

The role involves driving to patient homes, coordinating schedules, and assisting with exams, phlebotomy, and vaccinations under provider supervision.

Qualifications

• Certified/Registered Medical Assistant in applicable state or valid NREMT-A/Paramedic.
• 3 years of MA experience required for MA candidates.
• 1 year of pre-hospital EMT experience required.

Responsibilities

• Act as community-based medical assistant supporting primary, transitional, and acute care.
• Drive to patient homes and community locations for visits.
• Manage CB-NP schedule and follow-up appointments.
• Take vitals and perform age/condition-appropriate screenings.
• Review and reconcile medications; finalize medication reconciliation.
• Assist provider with exams and minor procedures.
• Perform phlebotomy and collect lab specimens.
• Prepare and administer medications/vaccines with provider authorization.
• Document medical history and communicate via teach-back.
