Job summary

AHRC Nassau in Seaford, NY seeks Direct Support Professionals to provide ongoing support to individuals in the Community Living Services program, helping maintain a high quality of life through self-care, health, safety, and daily routines. You will mentor new staff, follow treatment plans, document progress, and coordinate with an inter-disciplinary team.

A valid NYS driver's license is required, and a high school diploma is preferred.

Qualifications

• High School Diploma or equivalent preferred.
• Must submit a valid NYS driver's license and maintain qualified driver status.
• Ability to communicate and understand written and oral directives/information

Responsibilities

• Provide ongoing support to individuals to maintain a high quality of life.
• Assist with self-care, health, safety, household chores, socialization, and wellbeing.
• Support participation in the community and build natural supports and relationships.
• Document treatment plans and progress notes to track independence and wellbeing.
• Provide transportation to medical appointments, shopping, and activities.
• Mentor new Direct Support Staff by training and reviewing manuals.
• Attend House Meetings and maintain certifications and training expectations.
• Perform duties as assigned by supervisor.
