Position Purpose: Maintain collaborative relationships with Health & Family Services, government agencies, community resource and advocacy groups, to build additional community support for current and potential enrollees.
Additional responsibilities for Trillium Community Health Plan / Oregon Market:
Must Haves
Remote 80% and 20% travel for field/office meetings and trainings.
Education/Experience: Bachelor’s degree in Social Work, Nursing, or related field. 5+ years of social services or advocacy experience preferably in Medicaid environment.
License/Certification: LCSW or RN license preferred.
For Trillium Community Health Plan / Oregon Market:
Education/Experience: Bachelor’s degree in Social Work, Public Health, related field, or equivalent combination of skills, experience, and education that provides the applicant with the knowledge, skills, and ability required to perform the job. 5+ years’ experience in healthcare preferably in Medicaid environment, or community-based organizations including working with underserved communities, working with leadership, facilitating meetings, measuring, and monitoring programs, evaluation activities and working with multi-disciplinary, multi-cultural teams. Knowledge and demonstrated understanding of Traditional Health Worker roles across variety of settings including but not limited to a health system, social service, substance use disorder treatment, mental health, community-based organization.
License/Certification: Oregon Certified Traditional Health Worker preferred.
Pay Range: $26.50 - $47.59 per hour.

Actual pay will be adjusted based on an individual's skills, experience, education, and other job-related factors permitted by law. Total compensation may also include additional forms of incentives.
