Job summary

Devereux Advanced Behavioral Health is seeking a Community Integration Specialist to coordinate services for individuals in the CAAPP program in the Greater Pittsburgh area. The role involves ensuring service authorizations, monitoring outcomes, and maintaining documentation in the EMR system.

Your work supports person-centered care and quality outcomes within a collaborative, multidisciplinary team environment. A bachelor’s degree and relevant experience are required.

Qualifications

  • Two years’ experience working with adolescents and/or adults with autism.
  • Experience with ABA methods, case management or counseling preferred.
  • Proficient in Microsoft Office; data collection and analysis experience noted.
  • Strong organizational, and oral/written communication skills.

Responsibilities

  • Coordinate and oversee services for assigned individuals to ensure quality outcomes and person-centered care.
  • Complete intake assessments, maintain client records, and ensure timely documentation in EMR.
  • Monitor progress, collect data, and prepare treatment updates and reports.
  • Provide direct service coverage when needed to ensure continuity of care.
  • Ensure compliance with agency policies, licensing and regulatory guidelines.

Position: Community Integration Specialist
Compensation: $16.40-$18.70 per hour
Schedule: 37.5 hours a week (Full Time)
Location: Greater Pittsburgh Area
JOB SUMMARY:

Coordinates and facilitates access to services to service recipients in the Community Adult Autism Partnership Program (CAAPP). Monitors service delivery, utilization and team cohesiveness and evaluates and uses data to support the continuing need for services. Ensures that authorizations for services are obtained and revised . Establishes and maintains community-based sites to be utilized as vocational opportunities for assigned individuals.

  • Monitor progress, collect and analyze data, and prepare required reports and treatment updates.

  • Provide direct service coverage when needed to ensure continuity of care.

  • Encourage individuals to build independence, self-control, responsibility, and healthy interpersonal relationships.

  • Implement therapeutic strategies that enhance participation in daily living, social, and community activities.

Qualifications
EDUCATION:
  • Bachelor’s degree in Social Work, Social Science, Vocational/Rehabilitation Education, Psychology, Education, or related field required.
EXPERIENCE:
  • Two years’ experience working with adolescents and/or adults with an autism spectrum disorder.

  • Experience with ABA instructional methods, case management and/or counseling experience preferred.

  • Must be able to demonstrate excellent customer service.

  • Must be able to adapt to rapidly changing demands of the position, customer needs, and the organization.

  • Proficient computer skills, mastery of Microsoft Office Applications, experience with data collection/analyzing platforms (Radar, Pro-Filer, and/or other internal and external software applications pertinent to the position).

  • Strong organizational, oral and written communication skills.

  • Requires ability to work independently, prioritize, manage multiple tasks simultaneously, and adhere to established guidelines. Valid driver’s license must be maintained.

PHYSICAL REQUIREMENTS:

Regularly requires walking, standing, stooping, bending, repetitive wrist/hand movement, and pushing/pulling (1/2 to 3/4 of the workday). Often requires sitting. May require lifting up to 50 lbs. Range of hearing and vision sufficient to operate standard office machines and communicate with co-workers and stakeholders in person and via telephone. Free of dependency and abuse of illegal or illicit drugs, alcohol, inhalants, or other chemical substances or controlled substances. Seldom drives a Devereux vehicle or van. Frequent local travel. Regularly works inside in areas that are adequately lighted and ventilated (1/2 to 3/4of work time). Some fluctuation in temperature. May work outside or in wet conditions (<1/4 of work time).
